Dear Registrant,

 

You are participating in the whole man ministry Community Back to school rally. 

 

Release of liability statement: 

In exchange for participating in all activities organized by Whole Man Ministry 1241 E. 54th street indianapolis IN 46220, and or use the property facilities and services through Whole Man Ministries. 

 

I agree for myself and for members of my family to the following: to follow all directions and signs, assumption of risks and release, indemnification and any fees for damages that I may cause.

REGISTRATION STATEMENT 2

I, individually and for my Child and my child's other parent, further do hereby indemnify and hold harmless said releases against any and all losses, and/or expenses (including but not limited to legal fees) that they may incur as a result of claims made as a result of my child's participation in the activities.
